We are accepting new patients
To register with the surgery:
- fill out an adult new patient registration form online if you are 16 years old or over
- fill out a child new patient registration form online if you are under 16 years old or wish to register your child
- visit reception from 10am to 6pm and collect a paper registration form
When you register, it’s helpful to have your NHS number (but not essential). You can use the NHS website to find your NHS number.
Do you live out of area?
We are accepting registrations of patients who live outside our catchment area on a case by case basis.
If you work nearby or you are just keen to access our services you can do so by completing our Out of Area Registration form.
You can find out if you need to complete this form by checking our catchment area.
IMPORTANT : Registering with a GP surgery outside your local area can affect the NHS services that you can access including:
- home visits and urgent care from your GP, including out of hours care
- community services, such as physiotherapy and district nursing appointments
Ask us to explain any alternative arrangements for these services when you register. Sometimes we may recommend registering closer to your home
